We accept cash and check payments, PayPal payments using bank or credit card (with 3.5% fee added) and Venmo. Cash refunds will not be given under any circumstances. A $10 late fee will be added if your child comes on the first day of class without full payment. Participants that have not yet paid for class will NOT participate in trainings. There will be NO REFUNDS or CLASS CREDITS awarded for INDOOR GROUP CLASSES due to limited space that you have reserved. Please be sure of your program commitment when you sign up for INDOOR GROUP programs. Effort is made to email participants, as well as post on our CDS Facebook page at least one hour prior to class start if we have to cancel due to hazardous weather or emergency issue. For the safety of your child, never leave your child without making sure that a coach is present, in the event you may have missed a class cancellation message. Personal Training and "Create Your Own Group" Training Clients must send an email at least 24 hours before the start of session and is required for session credit to be granted. If any participant of a "Create Your Own Group" misses a session but others in their group still attend, the individual that missed forfeits that session. Personal Training sessions expire as follows: 13-25 sessions purchased expire within 150 days of purchase; 12 or fewer sessions expire within 90 days of purchase. Expiration is waived for medical reasons with a doctor's note. Please complete and return prior to beginning any training with CDS.

By signing below, I acknowledge that I understand the risks of athletic training and release Creative Dynamic Sports, LLC, its staff and facility owners from any liability related to injury, accident or harm. I certify that my child is physically able to participate and assume all risks associated with training.
